After completing undergraduate training at Southern Methodist University, I attended the University of Texas Medical Branch at Galveston. Following graduation from medical school, I was an intern and resident at the University of Texas Southwestern Medical School in Dallas in the Department of Orthopedics for five years. During my residency, I spent time in the United Kingdom studying joint replacement in general and attending a shoulder clinic at the Royal National Hospital in London. In 1983, upon graduating residency, I did a fellowship in reconstructive surgery of the shoulder under the mentorship of Charles A. Rockwood at the University of Texas Health Science Center at San Antonio. I have been in continual practice with an interest in shoulder and elbow surgery since January of 1984 at the W.B. Carrell Memorial Clinic. The W.B. Carrell Clinic was founded in 1921 by Dr. W.B. Carrell, who simultaneously founded the Texas Scottish Rite Hospital. It is the second oldest orthopedic group in the southern part of the United States of America. While I have been in private practice all this time, I have had an opportunity to teach and publish. My academic title is Clinical Professor of Orthopaedics, The University of Texas Southwestern Medical School. The bulk of my teaching responsibilities over the last years have been to my own shoulder fellows, which now number approximately 38. Over the last 34 years, in addition to performing nearly 20,000 operations, and caring for many more patients conservatively, I have had an opportunity to publish peer reviewed articles, book chapters, and make presentations both at a national and international level as outlined in my curriculum vitae (Appendix A). I have been blessed with many opportunities and honors; the one of which I’m most proud, is having been elected President of the American Shoulder and Elbow Surgeons. I am the author of a book entitled Rotator Cuff Disorders

Dr. Cristin John Mathew, is a Board Certified Orthopedic Surgery and Sports Medicine Specialist at All American Orthopedic & Sports Medicine Institute in Houston, Texas. He is also a Fellow of the American Academy of Orthopaedic Surgeons and American Association of Hip and Knee Surgeons. Dr. Mathew received his doctorate at the Edward Via College of Osteopathic Medicine in Blacksburg, Virginia. While in Blacksburg he worked with the Virginia Tech program, providing him an experience that fostered his passion for sports medicine and hip and knee surgery. He later completed his residency in Orthopedic Surgery at Northwell Health in New York. Dr. Mathew subsequently moved to Houston, Texas, and participated in a year of fellowship training in Sports Medicine and Arthroscopic Surgery at Houston Methodist Hospital. During his fellowship, Dr. Mathew served on the medical staff of the 2017 World Series Champions Houston Astros. In this position, he trained under many exceptional surgeons and used his sports medicine knowledge to help elite athletes improve their performance on the field. He has also served on the medical staff of Rice University Athletics, the Houston Rockets, and the Houston Texans. He is currently the team physician for the Houston Havoc Ultimate Team and several local rugby clubs, high schools and national rugby tournaments. Notably, he serves as the team physician for the National Collegiate Rugby Team and travels with the elite squad for international tours. He continues to stay active in mentoring young students and physicians through his academic affiliations with University of Texas Medical Branch and University of Houston Medical School. He has worked on several publications and continues to assist in several clinical research projects. He also serves as a consultant and assist in surgeon education. Dr. Mathew's practice focuses primarily on hip and knee pathology with treatments extending from complex joint preservation to muscle sparing joint reconstruction surgery.

Dr. Nicholas Sgaglione is Mauri Martocci Endowed Professor and Chair of the Northwell Health System Department of Orthopedic Surgery and at the Zucker School of Medicine at Hofstra / Northwell. He also serves as the Senior Vice President for Orthopaedics for the Northwell Health System and Executive Director of the Northwell Orthopaedic Institute in New York. Dr. Nicholas Sgaglione has been in clinical practice for 35 years in Great Neck, New York. He graduated from the Mount Sinai School of Medicine and completed his residency in orthopaedic surgery at the Hospital for Special Surgery in New York followed by a fellowship in sports medicine at the Southern California Orthopaedic Institute in Los Angeles. Dr. Sgaglione has been recognized in Best Doctors of New York State and Best Doctors of America since 2007. He served on the Board of Directors for the Arthroscopy Association of North America (AANA) beginning in 2008 and was President of AANA, the largest orthopaedic specialty society from 2012- 2013. Dr. Sgaglione was elected to the American Orthopaedic Association in 2006, a prestigious national society for leadership, education and research. He served as the chair of the Arthroscopy Association of North America Education Foundation as the chair of the Journal of Arthroscopy Board of Trustees. Dr. Sgaglione has also served as the New York State delegate for the American Orthopaedic Society of Sports Medicine and is an active member of the American Academy of Orthopaedic Surgeons, American Orthopaedic Society of Sports Medicine, International Cartilage Regeneration and Joint Preservation Society, International Society of Arthroscopy, Knee Surgery and Orthopaedic Sports Medicine and the New York Society of Orthopaedic Surgeons. He has served on the editorial board of several medical journals and is the editor of three orthopaedic textbooks and more than 100 published articles and book chapters on knee, shoulder, ankle and cartilage surgery. Dr. Sgaglione specializes in sports medicine and serves in numerous roles outside of Northwell Health, including the medical director for Hofstra University Athletics, an honorary police surgeon for the NYPD, and a consultant to the United States Ski Team and the US Professional Golf Association. He has also been the team physician for the United States Merchant Marine Academy for the past 35 years and the team medical director for the New York Lizards (2014 – 2020).
