Journey of a Pediatric Orthopedist - Contribution to the Osteopathic Profession
There is extensive crossover between degenerative spine and degenerative hip pathology. As orthopedic surgeons, it is critical we differentiate these conditions in to maximize therapeutic benefit with our treatment modalities. This lecture will dive further into work-up, treatment options, as well as historical data in these complex patients. Additionally, I will present some of my own cases that directly involve patients with both lumbar and hip degenerative conditions. We will discuss nonoperative treatment modalities, focusing from the lumbar region, and working our way distal towards the hip joint. We also discussed current surgical treatment modalities surgical and co-surgeon management of these patients.

The utilization of hip arthroscopic procedures for joint preservation is on the rise. One study shows rates have increased from 3.6 per 100,000 in 2005 to 16.7 per 100,000 in 2013. After the advent of pain ratings being the “fifth” vital sign in the early 90’s, there has been increased attention paid to pain control during patient care. Currently, there is not a clear consensus on the most effective pain control strategy for hip arthroscopy patients postoperatively although several strategies have been reported on. The most reported on methods include oral pain medications pre and postoperatively, peripheral nerve blocks, intra-articular injection, or local anesthetic infiltration. Here, we will review these strategies and highlight strengths and weaknesses of each in a review of the currently published literature.

After completing a graduate degree in Bioethics from the University of Pennsylvania, I completed medical school, internal medicine residency, and PCCM fellowship at New York University, where I was exposed to unprecedented critical care crises including Hurricane Sandy and surges in the COVID-19 pandemic; the latter of which was recognized by induction into the Gold Humanism Honor Society. I subsequently finished a second fellowship in Palliative Care at Mount Sinai in New York City to better facilitate goal-concordant care and conduct complex goals of care discussions near the end-of-life. I have a passion for bioethics, and in addition to having served on numerous hospital bioethics committees, taken leadership roles at the local and national level, I remain committed to incorporating my bioethical knowledge to further patient care as well as undergraduate & graduate medical education.
